A phase 3 clinical trial in the New England Journal of Medicine shows baxdrostat, an aldosterone synthase inhibitor, significantly lowers blood pressure in resistant hypertension patients, with around 40 percent reaching healthy levels.
The latest issue of the New England Journal of Medicine published the phase 3 clinical trial results for the new antihypertensive drug baxdrostat. The trial enrolled 794 patients with treatment-resistant or uncontrolled hypertension, randomly assigned to receive 1 milligram, 2 milligrams of baxdrostat, or placebo for a 12-week course. Results showed that patients taking 1 milligram had average systolic blood pressure decrease of 14.5 mmHg, and those taking 2 milligrams decreased 15.7 mmHg, both significantly better than the placebo group's 5.8 mmHg. Notably, approximately 40% of treated patients' blood pressure recovered to healthy levels, far exceeding the placebo group's less than 20% proportion. Baxdrostat is an aldosterone synthase inhibitor that lowers blood pressure by inhibiting adrenal secretion of the aldosterone hormone, with a mechanism completely different from existing first-line antihypertensive drugs. Researchers noted this target is particularly effective for patients whose hypertension is caused by excessive aldosterone secretion, a considerable proportion of the treatment-resistant hypertension population. Regarding safety, a small number of participants developed hyperkalemia, requiring clinical monitoring. The US FDA has placed the drug in priority review, with approval targeted for the second quarter of 2026. Given the global hypertension patient population exceeds one billion, with considerable treatment-resistant cases, these trial results are viewed by the medical community as an important breakthrough.
